Raymond Hallsworth walked away from his home in the 200 block of J Street near Alameda Creek at about 7:45 p.m. Saturday, police said.
Police said Hallsworth is 5 feet 8 inches tall, has a thin build, brown hair, brown eyes and a goatee. He was last seen wearing a green bicycle helmet, tan and white jacket, black sweatshirt and black pants.
Police consider Hallsworth an at risk missing person because he takes medication and had become disoriented today, police said.
Hallsworth may not respond to his name or be able to communicate, police said.
Police are asking anyone who sees Hallsworth to immediately call the Fremont Police Department.
